Citrobacter Endophthalmitis after Intravitreal Bevacizumab in a Patient with Asymptomatic Recurrent Dacryocystitis

Synopsis

Concurrent nasolacrimal irrigation with surgical vitrectomy for a Citrobacter Koseri vitreous abscess reveals an uncommon source for post-injection endophthalmitis.  This surgical video reveals  matching extensive fibrinous stranding in both the presumed source compartment and the abscess compartment.  Unusual causative organisms in post-procedural endophthalmitis should trigger questioning for possible source compartments of infection, such as the nasolacrimal system.
